Put each form in the right channel

Send sales inquiries to your sales channel and support requests to your support channel. Each form has its own destination.

Choose the answers your team sees

Select the form answers to include. Messages also contain the form name, submission details, and a link to the full response.

Follow View submission to the response in Maxforms. Your existing Maxforms access controls still apply.

Installation

Connect Slack to your form

Start in your Maxforms account. You can connect Slack while setting up a form, then choose exactly where its responses go.

1. Sign in and open your form
Sign in to Maxforms or create an account. Open the form you want to connect and select its Integrate tab.
2. Connect your Slack workspace
Find Slack, select Connect, then Connect New Account. Choose your Slack workspace in the window that opens and approve the requested permissions.
3. Choose the channel and fields
Back in Maxforms, select the connected account and a public channel. Review Fields to Send, then select Submit to save the integration.
4. Confirm your first notification
Publish your form and submit a test response. Check the Slack channel, then follow View submission to open the response in Maxforms.

Your Slack workspace may require an administrator to approve installation. A new connection does not enable notifications until you configure Slack for a form.

Permissions with a clear purpose

Post messages: send the submission fields you choose.
List public channels: show channel choices during setup.
Join public channels: join your selected channel when sending a notification.

Private channels and direct messages are not supported. Choose fields appropriate for everyone in the destination channel.

AI-assisted forms need your review

Maxforms offers optional AI-assisted form creation. AI-generated content may be inaccurate; review it before publishing.

The Slack integration posts selected submission fields and details. It does not generate AI replies or summaries in Slack.

Notifications report new submissions. Later booking, payment, or subscription changes do not send another message.

Before you connect

Questions about Maxforms for Slack

Do I need a Maxforms account?

Yes. Create an account or sign in, then connect Slack from Integrations. You also need permission to install apps in your Slack workspace.

Can I send submissions to private channels or direct messages?

The integration supports public channels only. Private channels and direct messages are not available.

Does connecting Slack start posting every form?

A new connection does not enable every form. Configure Slack on each form and choose its channel and fields. Reconnecting a disconnected workspace reactivates its existing form integrations, so review their settings before collecting more responses.

Does Maxforms read conversations in Slack?

This integration requests permission to post messages and to list and join public channels. It does not request conversation history or message-reading permissions.

Does the Slack integration generate AI responses?

No. Slack notifications contain selected form answers and submission details. Optional AI-assisted form creation happens in Maxforms, and its output needs your review.

What happens if a Slack delivery fails?

The response remains in Maxforms. Check the integration delivery log, fix the channel or connection, and confirm a new submission arrives. Reconnecting does not replay missed messages.

Can I stop notifications or disconnect Slack?

Yes. Pause Slack on one form from its Integrate tab, or disconnect the account from Integrations. Uninstalling Maxforms in Slack also disconnects it.
